Supplements and anticoagulation: what actually interacts?

A patient on warfarin comes in with a supratherapeutic INR. No dose change, no new prescriptions, no illness. On closer questioning, she started a "brain health" supplement a few weeks ago — ginkgo biloba, 120 mg daily, recommended by a friend. She never mentioned it, because she didn't think of it as a drug.

This scenario repeats constantly. Most patients on anticoagulants who use herbal or dietary supplements don't volunteer that information unless asked directly. Understanding which supplements matter — and why, based on the peer-reviewed evidence — is more useful than memorizing a long "avoid" list.

Two different mechanisms, two different problems

It helps to sort supplement interactions into two buckets, because they push in opposite directions clinically:

  • Additive bleeding risk — supplements with their own antiplatelet or anticoagulant activity that stack on top of the prescribed drug.

  • Reduced anticoagulant efficacy — supplements that induce the enzymes or transporters that clear the anticoagulant, lowering drug levels and raising clot risk.

The first group is the one most clinicians think of. The second is easier to miss, and arguably more dangerous, because the patient looks "fine" until they present with a stroke or a clot.

Ginkgo biloba

Inhibits platelet-activating factor (PAF)

Genuinely mixed. A large VA warfarin cohort found a significantly increased bleeding hazard (HR 1.38) with concurrent use, and a 2025 retrospective analysis found ginkgo interactions were significantly associated with bleeding risk and abnormal coagulation. But a systematic review of controlled trials using standardized extract (EGb 761) found no significant effect on hemostasis or on the safety of coadministered aspirin or warfarin, and a randomized crossover trial in 24 stable warfarin patients found ginkgo did not change warfarin dosing requirements. Treat it as a real but inconsistently reproducible risk.

Fish oil / omega-3

Alters platelet membrane composition and eicosanoid balance

More reassuring than commonly assumed. A retrospective cohort of 573 warfarin patients found no significant difference in time-in-therapeutic-range or bleeding events between fish/krill oil users and non-users. A study of 182 patients on high-dose fish oil (mean 3g) plus aspirin and clopidogrel found no significant increase in major bleeding versus aspirin/clopidogrel alone. A systematic review and meta-analysis of randomized trials similarly found no significant increase in bleeding risk with omega-3 supplementation.

Vitamin E

Inhibits activation of vitamin K-dependent clotting factors and platelet aggregation

Dose-dependent and mechanistically well-supported. Controlled studies show doses of 100–400 IU/day produce no significant change in coagulation parameters in warfarin patients, while case-level and cohort data link higher doses to enhanced anticoagulant effect and bleeding risk. A retrospective cohort study found an association between vitamin E serum levels and bleeding events in patients on oral anticoagulant therapy.

Garlic (concentrated supplements)

Inhibits platelet aggregation, dose-dependently

Established antiplatelet effect; reviewed among supplements with documented bleeding-risk potential in combination with antiplatelet and anticoagulant therapy.

Curcumin, ginger, ginseng

Variable antiplatelet/anticoagulant activity

Flagged in peer-reviewed reviews as supplements with anti-inflammatory vascular activity that also increase bleeding risk when combined with antithrombotic therapy, though the evidence base is smaller than for ginkgo or garlic.

Bucket 2: reduced anticoagulant efficacy

This is the one worth emphasizing to patients and colleagues, because it's counterintuitive — "natural" doesn't mean "makes the blood thinner."

  • St. John's Wort is the clearest example. It's a well-documented inducer of both CYP3A4 and intestinal/hepatic P-glycoprotein. A pharmacokinetic study in healthy volunteers showed St. John's Wort extract significantly decreased rivaroxaban exposure with a proportional decrease in pharmacodynamic effect. For warfarin and other CYP3A4/P-gp substrates, the same induction mechanism accelerates clearance and lowers drug levels. A comprehensive review of oral anticoagulant-herb interactions specifically flags St. John's Wort as a potent dual inducer that may reduce DOAC effectiveness. The practical message: this combination should generally be avoided, not just monitored.

  • For warfarin specifically, consistency of vitamin K intake matters more than avoidance — green leafy vegetables don't need to be eliminated, they need to be kept steady so dosing can be titrated around them.

  • Coenzyme Q10 has a chemical structure related to vitamin K. A peer-reviewed review of clinical herb-warfarin interactions describes case reports of reduced warfarin effect (INR dropping, e.g., from 2.5 to 1.4) with concurrent CoQ10 use, though a randomized placebo-controlled crossover trial in 24 stable warfarin patients found no significant effect on INR or warfarin dose at 100 mg/day. Evidence is mixed and dose-dependent.

The practical approach

Ask about supplements the same way you ask about medications — by name, not as an afterthought.

A few habits that catch most of the clinically relevant interactions:

  • Specifically ask about ginkgo, vitamin E, garlic, curcumin, and St. John's Wort at every anticoagulation visit — most patients won't offer this information unprompted.

  • For warfarin patients, an unexplained INR shift is a supplement question before it's a "warfarin resistance" workup.

  • A landmark review on herbal medicines and perioperative care recommends discontinuing garlic at least 7 days and ginkgo at least 36 hours before surgery; broader consensus guidance from perioperative societies recommends stopping most herbal supplements 1–2 weeks before elective procedures given variability in formulation and pharmacokinetics.

  • Treat St. John's Wort and DOACs as a combination to actively avoid, not just monitor — the efficacy loss is the bigger danger than the bleeding risk.

None of this requires memorizing every herb-drug interaction table. It requires treating supplements as pharmacologically active substances in the history — because to your patients' clotting factors, that's exactly what they are.
